From Dave Stroud, MPA Committee MPA Adjudicator Form - This is the adjudicator form for MPA events that the MPA committee composed. Soon the committee will be asking you for input. Official NMA Music Assessment Form - This is the form that our MPA committee used as inspiration for the MPA Adjudicator Form above. Grade 123 MPA Recommendation Form - This is the form you use to recommend that a composition be added to the official MPA Grade 1, 2, or 3 list. Note: This should be printed on 8.5 X 14 paper. Grade 456 MPA Recommendation Form - This is the form you use to recommend that composition be added to the official MPA Grade 4, 5, or 6 list. Note: This should be printed on 8.5 X 14 paper.
